Do You Have an Education, Health & Care Plan (EHCP)?
If you answered yes, the College's Additional Learning Support (ALS) team will need to review your EHCP before a conditional place can be offered.
Have you ever been enrolled in Alternative Provision (EG: PRU)
At school, do you have any extra support for examinations (EG: Extra Time, Reader or Scribe)
Are you a 'Young Carer' (aged 18 or under) for a parent, sibling, child with a disability, etc OR Are you an official 'Looked After Child' (LAC)?
Are you involved with any external agencies, such as Social Services or CAMHS
Do you have any Disability/Ongoing Medical Condition the College needs to be aware of?
Do you have any unspent criminal convictions or convictions that can never become unspent?

If you answered YES, the College will send you a form to complete and return. The College will then review the details and advise if you are able to enrol with the College.
